CONSENT TO USE THE IMAGE

I, the undersigned ………………………………………., pursuant to art. 81 section 1 of the Act on Copyright and Related Rights of 4 February 1994, consent to the free use by Ms. Iga Majewska, conducting business under the brand "Warsaw Pilates Iga Majewska" (ul. Jana Kazimierza 12/35, 01-248 Warsaw, NIP: 821-256-38-35) and JOY IN ME SC (ul. Ogrodowa 48/54, 00-876 Warsaw, NIP: 5272882838), of my image recorded in the form of photographs and video recordings, in connection with the conduct of workshops or trainings by Iga Majewska and JOY IN ME SC under the name "Training pod śniadanie" (Open-air training). The consent is granted without territorial restrictions, with the right to grant further consents by Iga Majewska to third parties, including:

  1. posting, sharing and disseminating photos or videos on the Internet (including on Warsaw Pilates websites, on subscription platforms with online training, on YouTube and on social media) in order to promote the products, brand and activities of Warsaw Pilates and the brands JOYINME, Cosibella Corner, Nick's, Felix, Mudii, which are partners of the above-mentioned event; and

  2. posting, sharing and distributing photos or videos in all Warsaw Pilates promotional and informational materials, documents, posters, brochures, leaflets, newspapers, magazines and all other electronic and traditional large- and small-format media in order to promote the products, brand and activities of Warsaw Pilates and its partners' brands; and

  3. reproduction and recording of photographs and recordings, including printing, reprographic, magnetic and digital recording techniques; and

  4. public exhibition, display, reproduction, broadcast and rebroadcast of recordings and photographs; and

  5. use of photos or video materials for advertising and promotional campaigns of Warsaw Pilates and sponsoring brands.

Consent is granted for the entire duration of the promotional activities in which the image was recorded.

Iga Majewska "Warsaw Pilates," as the controller of your personal data, informs you that your data will be processed for the purposes of promoting Warsaw Pilates' activities. You have the right to access and rectify your data, as well as to request its erasure and restriction of processing. Warsaw Pilates will respond to your request within 30 days of its submission and will cease processing the data if there are grounds to do so. The data may be transferred to event sponsors .

Personal data (image) will be processed for the duration of the promotional or marketing activities in connection with which the image was recorded. If you believe that we are processing your data unlawfully, please contact us at kontakt@warsawpilates.pl. You also have the right to lodge a complaint regarding the processing of your personal data with the President of the Office for Personal Data Protection.

Consent may be withdrawn at any time by sending an email to kontakt@warsawpilates.pl or by mail to the administrator's address.
